Academic Talent Tuition Waiver Application [pdf]- to be submitted by domestic MBA students to the MBA Office to apply for a tuition waiver. Applications currently cannot be completed online because a signature is required.

International Graduate Tuition Waiver Application [pdf] - to be submitted by international MBA students to the Office of International Studies. Applications currently cannot be completed online because a signature is required.

Waiting List/Override Form [htm] - to be submitted by MBA students to the MBA Office if they are being blocked from registering for a course because the computer does not recognize they have had the prerequisites for a course, or because the course is full.

Non-Major Enrollment Request Form [pdf] - to be submitted to the MBA Office by non-MBA students who are in other masters degree programs at Illinois State University and who would like to take an MBA course

Full-Time/Part-Time Status Change Form [pdf] - to be submitted by MBA students to the MBA Office if they were admitted on a part-time basis but wish to take more than two courses per semester, and to be submitted by MBA students to the MBA Office if they were admitted on a full-time basis but wish to take less than three courses per semester

Masters Plan of Study [pdf] - to be submitted by MBA students to the MBA Office at the beginning of their final semester, to be forwarded to the Graduate School.

Under "Deficiency Courses," list any MBA foundation courses you were required to complete (ACC 401, ECO 401, MQM 406, MQM 402, MKT 403, FIL 404, and/or FIL 405).

Remember also to file an Application for Completion of Graduate Degree and, if appropriate, a Commencement Participation Form.

Change in Plan of Study [pdf] - To be used if changes need to be made to an already submitted Master Plan of Study.

Application for Completion of Graduate Degree/Commencement Participation Form [pdf] - to be submitted by graduate students to the Graduate School at the beginning of their final semester, after they submit their Plan of Study.

A $30 fee must accompany the form. (Remember also to file the Commencement Participation Form, page 2 of the Application for Degree Completion document, if appropriate.)

Independent Study Form [pdf] - to be submitted by MBA students to the MBA Office if they wish to take an independent study.

Request for Transfer Credit [pdf] - to be submitted to the MBA Office in order to be forwarded to the Graduate School, if an MBA student takes core or elective courses at another university. (advance approval must be obtained from the MBA advisor)

Graduate Assistant Course Load Waiver Request Form (PERS 938) [doc] - to be submitted to the MBA Office in order to be forwarded to the Graduate School, if the student is a Graduate Assistant and will be taking less than 9 hours of credit in a semester

Reduced Course Load Form [pdf] - to be submitted by international MBA students to the MBA Office, to be forwarded to the Office of International Studies, if they will be taking less than 9 hours of credit in a semester.

Curricular Practical Training Application [pdf] - to be submitted by international MBA students to the MBA Office, to be forwarded to the Office of International Studies, if an MBA student would like to pursue work which counts for academic credit toward their degree

Optional Practical Training Application [pdf] -to be submitted to the MBA Office, to be forwarded to the Office of International Studies, if an international MBA student would like to pursue work which will not count for academic credit
